[SASAG] load-balancing equipment

I have need for a load balancer that can handle per-connection load
balancing for 5000 or so simultaneous connections balanced across 120 or
so machines, at a throughput rate of around 2Gbps.  Automatically keeping
track of which machines are up and which are down is a must.  Bonus points 
if they can be clustered.

Does anyone have any recommendations for hardware that does this?  
Anyone have any warnings about what *not* to use?
