Do you do some kind of SMTP look ahead to find out if the email address
actually exists, or maintain 2 user lists?

I've been using Postfix w/Amavisd-new (which itself uses SpamAssassin,
DSPAM, and ClamAV). I have Postfix configured to perform serveral
pre-queue checks, which then passes the mail on to Amavisd, to then pass
on to Exchange.
